29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a. On or about May 27, 2014, employees used on the exterior of the south side of the building a 20-foot fiberglass extension ladder that because of it's length the side rails did not extend 3 feet above the roof surface it was secured to a rigid support but no grasping device to assist employees in mounting and dismounting the ladder to and from the 6:12 pitch roof being sheathed, being exposed to a 14-foot 7-inch fall hazard.  b. On or about May 27, 2014, employees used on the interior of the southwest side of the building a 20-foot fiberglass extension ladder that because of it's length the side rails did not extend 3 feet above the roof surface it was secured to a rigid support but no grasping device to assist employees in mounting and dismounting the ladder to and from the 6:12 pitch roof being sheathed, being exposed to a 16 feet fall hazard.

29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   a. On or about May 27, 2014, employees worked on the 6:12 pitch roof attaching roof sheathing without the use of any conventional fall protection system, being exposed to 15, 16 and 18 feet fall hazards.   The Florida's Frame Pro, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 315480699, citation number 1, item number 1 and was affirmed as a final order on December 6, 2011, with respect to a workplace located at 13162 Atlantic Blvd., Jacksonville, FL 32225.
