29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded. On and before May 28, 2014, during a role change at the #4 piercing mill within the Gambrinus Steel plant, the employer exposed an employee to two floor holes, from which the employee could trip or fall, when requiring them to knock out the four wedges which hold the cap in place.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. a) On and before May 28, 2014, during a role change at the #4 piercing mill within the Gambrinus Steel plant, the employer exposed an employee to a fall hazard, of approximately 97 inches, when requiring them to knock the east cap wedge out which is next to an open side in the guardrail. b) On and before May 28, 2014, during a role change at the #4 piercing mill within the Gambrinus Steel plant, the employer exposed an employee to a fall hazard, of approximately 97 inches, when requiring them to knock the west cap wedge out which is next to an open side in the guardrail.
