Canton Ohio 44706, were exposed to a fall hazard of approximately 78 inches while knocking out and replacing the retaining wedges to the top shoe holder next to an opening in the guardrail.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about February 10, 2015, employees who perform bottom shoe change outs on the rotary sizer machine in tube mill number three, located at 2401 Gambrinus Ave., S.W. Canton, Ohio 44706, were not required to operate the energy isolating devices(electrical disconnect switches) to isolate the drive motor and inlet rolls prior to entering the rotary sizer to perform the shoe change.
