29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:     (a)  On May 28, 2014, at a jobsite located at 3107 3107 Erie Park, lot #78, NKC, MO., an employee was exposed to serious fall injuries from unprotected sides and edges, in that he was engaged in sheathing activities on a 12:12 pitch roof without using any Fall Protection Equipment. The fall was from 10 feet 4 inches up to approx. 27 feet to the ground below.      Abatement certification for this violation is not required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      (a)  On May 28, 2014, at a jobsite located at 3107 3107 Erie Park, lot #78, NKC, MO., an employee was exposed to serious fall injuries, in that the portable ladder used to access the roof did not extend at least 3ft above the upper landing surface. As a result, the employee was exposed to a fall of approx. 10 feet 4 inches to the ground below.     Abatement certification for this violation is not required.
