29 CFR 1910.23(e)(3)(iv): The anchoring of posts and framing of members for railings of all types were not of such construction that the completed structure was capable of withstanding a load of at least 200 pounds applied in any direction at any point on the top rail:  (a)  On or about June 4, 2014, and at times prior, employees were exposed to falls of greater than four feet from the platform around the head of the distributor where the guard rail on the northeast side was not adequately anchored such that it wobbled loosely. 29 CFR 1910.27(d)(1)(ii): Cages or wells shall be provided for fixed ladders of more than 20 feet (except as provided in subparagraph 5 of this paragraph) to a maximum unbroken length of 30 feet:    (a)  On or about June 4, 2014, and at times prior, employees were exposed to fall hazards from a fixed ladder on the south side of the grain elevator that was approximately 35 feet high that did not have a cage or a ladder safety device. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a)  On or about June 4, 2014, and at times prior, a hole in the bottom of the "Electrical Controls" box on the outside of the elevator building on the west side south of the garage bay, was open.   (b)  On or about June 4, 2014, and at times prior, the cover for an electrical box in the elevator motor control room adjacent to the dump pit, was not fully closed exposing a gap in which dust could accumulate.
