Manufactured Housing Enterprises, Inc. located at 09302 St. Route 6, Bryan, OH 43506; On or about June 24, 2014, the employer did not assure the makeshift scaffolding was maintained in a safe condition. Employees are required to use the scaffolding to perform various activities including siding and accessing the roof and are exposed to fall hazards of approximately 8 ft. in that the racking supporting the platforms were bent and damaged.

Employees were exposed to fall hazards of approximately 8 ft. while performing roofing and siding activities on platforms held up by damaged racking.
