29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. Manufactured Housing Enterprises, Inc., at a worksite located at  09302 US Hwy 6, Bryan, Ohio 43506:  On or about May 1, 2024,  In building 2, the employer did not ensure employees were protected from falls while  working near an unprotected edge on the  north side mezzanine area.  Employees performing these work activities were exposed to a fall of approximately 9 feet to a dirt surface below.  b. Manufactured Housing Enterprises, Inc., at a worksite located at  09302 US Hwy 6, Bryan, Ohio 43506:  On or about May 1, 2024,  In building 2, the employer did not ensure employees were protected from falls while working near an unprotected edge on the east side mezzanine area.  Employees performing these work activities were exposed  exposed to a fall of approximately 7 feet to a concrete surface below.

Employees tasked with operating pneumatic nailers/staplers, radial arm saws and table saws without the benefit of an employer assessment were exposed to eye, head, and bodily injuries.

Manufactured Housing Enterprises, Inc., at a worksite located at  09302 US Hwy 6, Bryan, Ohio 43506:  On or about May 1, 2024,  in building 2, partitions area, the employer did not ensure that each affected employee wearing prescription lenses were provided with eye protection that incorporates the prescription in its design. Employees operating pneumatic nailers were exposed to flying particles, wood chips, in that employees wearing prescription lenses were not provided with safety glasses or side shield attachments.  b. Manufactured Housing Enterprises, Inc., at a worksite located at  09302 US Hwy 6, Bryan, Ohio 43506:  On or about May 1, 2024,  in the saw shop, the employer did not ensure that each affected employee wearing prescription lenses were provided with eye protection that incorporates the prescription in its design. Employees operating pneumatic nailers and table saws were exposed to flying particles, wood chips, in that employees wearing prescription lenses were not provided with safety glasses or side shield attachments.

The 2024 300 log did not define the object that directly caused the injury to the employee.
