29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Fertilizer bin area: On or about June 19, 2014, employees engaged in the installation of a fertilizer handling/processing system at a grain elevator did not wear provided hard hats while in the construction area. This exposed them to the hazard of being struck by falling or swinging objects.

29 CFR 1926.102(a)(3): The vision of employees requiring use of corrective lenses in spectacles was not protected by goggles or spectacles of one of the types listed in paragraphs (i) through (iii) of this section: Fertilizer system construction area: On or about June 19, and time prior to, and employee engaged in the installation of fertilizer handling/processing system wore only non protective prescription eyewear. This exposed the employee to the hazard of being struck in the eyes flying debris generated by construction activities.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Top of the fertilizer bins: On or about June 19th, 2014, employees were installing a fertilizer handling system at a grain elevator. An employee working on top of the bins climbed over a bin top guardrails to an adjoining bin top guardrails without using any fall protection. This exposed the employee to the hazard of falling over 6 feet.
