29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Fertilizer Storage House - On or about November 6, 2014 employees standing on ceiling joists and sheets of plywood while installing a screw auger in the rafters of the building were not protected by guardrail systems, safety net systems or personal fall arrest systems, thus exposing employees to fall hazards of up to about 14 feet onto concrete ground. Industrial Ironworks, Inc. dba Adams Fertilizer Equipment Manufacturer was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1926.501(b)(1)], which was contained in OSHA inspection number 982186, citation number 1, item number 3 and was affirmed as a final order on November 3, 2014, with respect to a workplace located at 130 S. Arapaho, Hydro, Oklahoma 73048.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) Fertilizer Storage House - On or about November 6, 2014, the employer did not ensure that employees were trained in the proper use and operation of all components of personal fall arrest systems, in that employees were not knowledgeable regarding the limitation of their lanyards, which had snap hooks that were not designed to be attached to the lanyard webbing.
