29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: On or about November 19, 2014 and times prior thereto, employees of Industrial Iron Works, Inc. DBA Adams Fertilizer Equipment performed abrasive blasting at Building #12 located at or near 1503 S. Whitehead Drive in Dewitt, Arkansas. Supplied-air hood respirators manufactured by Clemco Industries Corporation were not stored properly to protect from contamination and dust, exposing the respirators to damage and employees to potential health hazards during respirator use.

DBA Adams Fertilizer Equipment performed abrasive blasting in Building #12 of the employer's facility located at or near 1503 South Whitehead Drive in Dewitt, Arkansas. The employer used an oil lubricated portable air compressor, Sullair 260 with serial number 004-138068 to supply breathing air. The compressor did not have a high-temperature or carbon monoxide alarm exposing employees to carbon monoxide hazards.
