29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 30, 2014, the employer failed to provide machine guarding on Press #2 for the following areas: a) the back side of the press by the log loader, preheat oven, and the electrical panels had an unlocked access door, b) the back side of the press by the ram had an unlocked access door, c) the back side of the press by the puller had an access panel not permanently mounted so that employees could access the area to perform maintenance and there was enough slack in the interlocked cable to allow access to the moving parts, and d) the back side of the press where the knurler is put into position had an unlocked access door.
