29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:  (a) On or about 7/15/14 - at Building 102, employees working on top of a Stryker vehicle were not protected from falling about 8-feet at the sides and back and 6-feet at the front.  (b) On or about 7/15/14 - at Building 103, employees working on top of a Stryker vehicle were not protected from falling about 8-feet at the sides and back and 6-feet at the front.

(b) On or about 7/15/14 - at Building 5 Machine Shop & Weld, the L4951 Plasma Cutter Lockout Procedure did not outline the rules and techniques to be utilized for the control of hazardous energy.  (c) On or about 7/15/14 - at Building 145, the energy control procedures for vertical and horizontal machining centers, such as but not limited, to L5190 Monarch PMC V750 Lockout Procedure did not outline the rules and techniques to be utilized for the control of hazardous energy.  (d) On or about 7/15/14 - at Building 147, the energy control procedures for lathes, such as but not limited, to HAAS Lathe CNC L6178 Lockout Procedure did not outline the rules and techniques to be utilized for the control of hazardous energy.

The U.S. Army Materiel Command was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 315969147, citation number 01, item number 001 and was affirmed as a final order on 2/16/12, with respect to a workplace located at 7 Frankford Ave, Bldg 16, Army Depot, Anniston, AL 36201.

(b) On or about 7/15/14 - at Building 5 Sheet Metal, the small roller machine was not guarded.  (c) On or about 7/15/14 - at Building 5 Sheet Metal, the large roller machine was not guarded.  (d) On or about 7/15/14 - at Building 5 Sheet Metal, the punch press was not guarded.  (e) On or about 7/15/14 - at Building 400 Machine Shop, the unused portion of the Spartan horizontal band saw blade adjacent to the control station was not guarded.
