29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: (a) On or about 12/16/16 - Building 432 baghouse, the employer did not ensure that respirators were cleaned and disinfected between uses. The presence of contaminants such as, but not limited to, cobalt dust, cadmium, nickel, zinc oxide, iron oxide, copper, manganese and chromium were detected by wipe samples collected on May 08, 2017.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Particulates-Not-Otherwise-Regulated (PNOR) listed in Table Z-1 in excess of the 8-hour Time-Weighted concentration of 15.0 mg/m3. Employee(s) were exposed to an airborne concentration of Chromium (metal and insoluble salts) listed in Table Z-1 in excess of the 8-hour Time-Weighted concentration of 1.0 mg/m3: (a) On or about 12/16/16 - Building 432 dust collector, a millwright employee conducting maintenance work was exposed to an airborne concentration of Particulates-Not-Otherwise-Regulated (PNOR) of approximately 19.1 mg/m3 or 1.27 times the PEL. Sampling was conducted on 05/08/17 for 220 minutes. Zero exposure was assumed for the remainder of the shift. The employee was wearing a Pureflo PF60ESM Powered Air-Purifying Respirator (PAPR) during the sampling period. (b) On or about 12/16/16 - Building 432 dust collector, a millwright employee conducting maintenance work was exposed to an airborne concentration of Particulates-Not-Otherwise Regulated (PNOR) of approximately 65.5 mg/m3 or 4.36 times the PEL. Sampling was conducted on 05/08/17 for 205 minutes. Zero exposure was assumed for the remainder of the shift. The employee was wearing a Pureflo PF60ESM Powered Air-Purifying Respirator (PAPR) during the sampling period. (c) On or about 12/16/16 - Building 432 dust collector, a millwright employee conducting maintenance work was exposed to an airborne concentration of chromium (metal and insoluble salts) of approximately 1.19 mg/m3 or 1.19 times the PEL. Sampling was conducted on 05/08/17 for 115 minutes. Zero exposure was assumed for the remainder of the shift. The employee was wearing a 3M Versaflo Powered Air-Purifying Respirator (PAPR) during the sampling period.

Sampling was conducted on 05/08/17 for 220 minutes. Zero exposure was assumed for the remainder of the shift. The employee was wearing a Pureflo PF60ESM Powered Air-Purifying Respirator (PAPR) during the sampling period. (b) On or about 12/16/16 - Building 432dust collector, a millwright employee conducting maintenance work was exposed to an airborne concentration of cadmium of approximately 16 ug/m3 or 3.2 times the PEL. Sampling was conducted on 05/08/17 for 115 minutes. Zero exposure was assumed for the remainder of the shift.
