The powered industrial truck that was found to have an unsafe condition was the Hyster forklift, Serial #U187MO2702E, in which the parking brake was not in working order.      b) Sheeting #2 Area - The employer did not ensure that all powered industrial trucks in need of service were removed from service until repairs could be made.  The powered industrial truck that was found to have an unsafe condition was the Toyota forklift, Model #FGC45, Serial #FGC45-11871, that had the rubber covers off the foot pedals making them slippery for the operators when operating the forklift.    c) Warehouse - The employer did not ensure that all powered industrial trucks in need of service were removed from service until repairs could be made.  The powered industrial truck that was found to have an unsafe condition was the Nissan forklift, #22, in which the parking brake was not in working order.
