29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    1.  On or about June 12, 2019 at the L2 extruder machine where employees were exposed to electrical hazards when a 480 volt fuse compartment did not have its covers shut. 2.  On or about June 12, 2019 below the breaker box located next to the exit door employees were exposed to electrical hazards when a metal conduit body cover was not completely closed. 3.  On or about June 12, 2019 at the L6 extruder machine where employees were exposed to electrical hazards when a 480 volt breaker fuse compartment did not have its cover shut.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    1.     On or about June 12, 2019 between the L2 and L6 extruder machines employees were exposed to electrical hazards when using an electrical breaker box that had an unused opening. 2.     On or about June 12, 2019 at the L4 extruder machine, employees were exposed to electrical hazards from an unused opening at the roll press control panel. 3.     On or about June 12, 2019 at the L4 extruder machine, employees were exposed to electrical hazards from seven unused openings on the control panel. 4.     On or about June 12, 2019 at the L4 extruder machine, employees were exposed to electrical hazards from an unused opening at the stairwell breaker box. 5.     On or about June 12, 2019 at the L6 extruder machine ,employees were exposed to electrical hazards from five unused openings at the roll press control panel. 6.     On or about June 12, 2019 at the L6 extruder machine, employees were exposed to electrical hazards from two unused openings at the control panel. 7.     On or about June 12, 2019 at the L6 extruder winder, employees were exposed to electrical hazards from two unused openings.  8.     On or about June 12, 2019 at the L6 extruder machine,  employees were exposed to electrical hazards from an electrical box that had an unused opening.
