29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On July 28, 2014 an employee wearing a complete personal fall arrest system was attached to the safety rope at the front side shoulder strap near his waist, exposing the employee to the hazards associated with wearing a personal fall arrest system incorrectly. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
