29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors: On or about July 28, 2014, employees were working on a rig site, Cactus Rig #143. On the crew hand trailer #23243, the cable coming out of the breaker box was running through a hole which lacked a rubber bushing to protect the cable from being damaged and eventually exposing the conductors. This condition exposed employees to electrical shock hazards.
