29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 8/11/2014, at the Tucker facility, employees cleaning the shaker machines and surrounding areas were not applying locks to the equipment in which they were cleaning. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On or about 8/11/14, at the Tucker facility, employees trained as "Affected" employees were performing "Authorized" activities of cleaning equipment in the production area but were not trained to lockout the equipment they were cleaning themselves. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
