29 CFR 1910.147(a)(2)(ii)(B): An employee is required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle, without the use of alternative protective measures. (a) On or about August 18, 2014, the employer failed to develop, document, and implement alternative means of protection for employees adjusting the packing machine during routine production, exposing them to amputation hazards from the piston and adjustment screw.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about August 18, 2014, the employer failed to ensure that the spacing between the piston and adjustment screw on the packing machine was guarded to prevent employees from accessing the area while the machine is in operation, exposing employees to an amputation hazard.
