29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). (a) On or about June 2, 2025, located at 726 S. 12th Street, Milwaukee, WI 53204, employees were exposed to amputation hazards associated with moving and rotating parts when conducting sanitation services on a Leland Model H-700DA Industrial food mixer. The employer did not ensure that energy control application steps were implemented as the Leland mixer was not shut down and turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)] were not performed.
