29 CFR 1910.176(b): Storage of materials created a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapsing: At the worksite: On or about August 26, 2014, the employer did not have the pipe on the pipe racks chocked or block to prevent shifting. Employees were exposed to struck-by and caught in between hazards.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from fall from heights: At the worksite: On or about August 26, 2014, the employer allowed the use of an anchor system for the emergency escape system that was not installed per the manufacturers requirements. Employee was exposed to the hazard of fall from heights.
