29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite: East and west exterior precast concrete walls - On or about 9/4/14, the employer did not provide a safe means of access to the platforms of the supported scaffolds by the use of portable ladders, hook-on ladders or other acceptable means.  Employees climbed the exterior structure of the walls to access the scaffold platforms.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite: Southeast and southwest sections of structure - On or about 9/4/14, employees were not provided with a fall protection system while conducting precast concrete operations from supported scaffolds.  Employees conducting the precast concrete operations were exposed to 10 foot falls.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Jobsite: Southeast and southwest sections of structure - On or about 9/4/14, employees who performed  precast concrete operations from the top edge of the wall, approximately 12 feet above ground level, were exposed to fall hazards without the use of a fall protection system.
