29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  Employees performing roofing work on a 6/12-pitch roof 6 feet or more above ground are not protected from 8ft-3in and 12ft-10in fall hazards by a fall protection system.  This violation was most recently observed on or about September 25, 2014, at a job site located at 4324 Queensway Dr., Jacksonville, Florida.   Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 447093, Other-than-serious citation number 1, item number 2 and was affirmed as a final order on June 27, 2012, with respect to a workplace located at 12957 Everett St, Jacksonville, Florida.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 931265, Repeat Serious citation number 1, item number 2 and was affirmed as a final order on November 6, 2013, with respect to a workplace located at 2276 Gabriel Drive, Orange Park, Florida.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 941605, Repeat Serious citation number 1, item number 1 and was affirmed as a final order on November 6, 2013, with respect to a workplace located at 1421 Chablis Court North, Orange Park, Florida.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 959679, Repeat Serious citation number 1, item number 1 and was affirmed as a final order on April 23, 2014, with respect to a workplace located at 96 Bayside Dr., Palm Coast, Florida.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about September 25, 2014, at the north side of the building, employees using pneumatic air nail guns to install shingles on a roof were not wearing eye protection, exposing themselves to being struck by flying debris or nails.  b.  On or about September 25, 2014, at the northwest side of the building, an employee using a Black and Decker circular saw to cut plywood on the ground was not wearing eye protection, exposing himself to being struck by flying debris.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 447093, Serious citation number 1, item number 1 and was affirmed as a final order on June 27, 2012, with respect to a workplace located at 12957 Everett St, Jacksonville, Florida. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a.  On or about September 25, 2014, at the north west side of the building,  the extension cord being used for the Black and Decker circular saw had the strain relief broken with exposed internal wiring.

On or about September 25, 2014, at the north side of the building, employees performing roof work on a steep-pitch roof 6 feet or more above ground were not properly trained in the use of personal fall arrest systems and were exposed to body injuries.
