29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).      On or about October 10, 2014, the employer did not ensure that each working level of the tubular welded frame scaffold was fully planked, exposing the employees to a fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.   On or about October 10, 2014, the employer did not ensure that guardrails were installed along all open sides and ends of the following scaffolds, exposing employees to fall hazards of approximately 10 feet and greater to the lower level:  a)  Scaffold 1, a two-tier scaffold, far west end of building. b)  Scaffold 2, a two-tier scaffold, up on the canopy, west end. c)  Scaffold 3, a two-tier scaffold, in the middle of the canopy. d)  Scaffold 4, a two-tier scaffold, closest to the center of the building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure,  personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.      On or about October 10, 2014, the employer did not ensure that a ladder was provided to gain access or egress to each working level of the scaffold system.  The employees climbed the cross-bracing and end rails to gain access exposing the employees to fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about October 10, 2014, employees performing masonry activities on a canopy, worked next to the edge without fall protection or approved fall protection systems exposing employees to an unprotected fall hazard of approximately 14 foot to the ground below.
