29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: On or about August 15,2016, at 2570 Blackmon Dr., Decatur, Georgia: a) Employees were preforming block masonry work from a scaffolding erected on legs that were supported by CMUs. Employer did not ensure that the tubular welded frame scaffolds had an adequate foundation, exposing employee(s) to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about August 15, 2016, at 2750 Blackmon Dr., Decatur, Georgia: a) Employees preforming block masonry work were exposed to a fall hazard of approximately 19 feet when they climbed the frame of a tubular welded frame scaffold erected at the west wall of Building 500. b) Employees preforming block masonry work were exposed to a fall hazard of approximately 19.3 feet when they climbed the frame of a tubular welded frame scaffold erected at the west wall of Building 600. Olympic Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 999898, citation number 1, item number 2, and was affirmed as a final order on 12/23/14, with respect to a workplace located at 9150 N. Tarrant Pkwy, North Richland Hills, Texas, 76182.

On or about August 15, 2016, at 2750 Blackmon Drive, Decatur, Georgia: a) The employees preforming block masonry work from a work platform of a tubular welded frame scaffold located at the west wall Building 700 Regions were exposed to a fall hazard of approximately 14 feet when employees were working without fall protection or nets. b) The employees preforming block masonry work from a work platform of a tubular welded frame scaffold located at the west wall Building 600 Zoë's Kitchen were exposed to a fall hazard of approximately 14 feet when employees were working without fall protection or nets. c) The employees preforming block masonry work from a work platform of a tubular welded frame scaffold located at the east wall Building 500 Sprouts were exposed to a fall hazard of approximately 14 feet when employees were working without fall protection or nets. Olympic Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(i), which was contained in OSHA inspection number 999898, citation number 1, item number 1/b and was affirmed as a final order on 12/23/14, with respect to a workplace located at 9150 N.
