At the construction site located at 110 N. Hamilton Road, Gahanna, Ohio, employees operated a Gradall material handler, model 534-C, serial 0388321, to load materials onto a roof that had a shattered side cab glass that created distortion of view, thereby exposing employees to a struck-by hazard.

Proframing Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 192137, citation number 1, item number 5 and was affirmed as a final order on April 12, 2012, with respect to a workplace located at 6175 Camden Center, Dublin, Ohio.
