The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment.  a) Building 7 and Fractionator Area  Employer's written mechanical integrity procedures did not memorialize the specific steps required for the inspections of rupture disks, level alarms, and interlocks.  Violation occurred on or about 10/21/14

29 CFR 1910.119(j)(4)(iv): The employer shall document each inspection and test that has been performed on process equipment. The documentation shall identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test.   The documentation of the inspection or test that has been performed on process equipment to maintain its mechanical integrity did not identify: the date of the inspection or test; the serial number or other identifier of the equipment on which the inspection or test was performed; a description of the inspection or test; and the results of the inspection or test:  a) Building 71 and fractionator Area  The written record of inspections for rupture discs PSE 133 and PSE 146 in the methanol recovery process did not include all required information.  Violation occurred on or about 10/21/14.  b) Building 71 and Fractionator Area  The written record of inspection s for level alarms LT110 and LT102 for the fractionator or for tanks T7115, T7119, and T714D did not include all required information.  Violation occurred on or about 10/21/14.  c) Building 71 and Fractionator Area  The written records of inspections for the interlock closing FV101 and FV116 in the methanol recovery process did not include all required information.    Violation occurred on or about 10/21/14.
