OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible aluminum dust deflagration, explosion, or other fire hazards while working at or near the thermal spray operation:         (a)  Major Metals Company, 844 Kochheiser Road, Mansfield, Ohio:  On or about October 23, 2014, the employer did not implement appropriate safeguards to eliminate or control the hazards created by the dust generated by the thermal spray system. Aluminum dust generated by the thermal spray system on tube mill #3 accumulated in the ventilation duct creating fire and explosion hazards.      Among other methods, feasible abatement methods to correct this hazard is to develop and implement a routine cleaning of the duct, construct the duct without capped outlets, pockets, or other dead-spaces, and provide air volume and air velocity necessary to keep the duct interior clean and free of residual material.  Applicable abatement guidance can be found in the National Fire Protection Association (NFPA) 484, Standard for Combustible Metals 2015 edition, section 9.2.2.8, 9.4.4, 9.4.10.2, and 9.4.10.5.2, 12.2.3, the NFPA 91, Standard for Exhaust systems for Air Conveying of Vapors, Gases, Mists, and Noncombustible Particulate Solids, 2010 edition, section 4.4.7, the Factory Mutual (FM) Loss Prevention Data Sheet #7-76 Prevention and Mitigation of Combustible dust Explosion and Fire 2014 edition, section 2.3.4.5, the Aluminum Association F-1 Guidelines for Handling Aluminum Fines Generated during Various Aluminum Fabricating Operations, ANSI/American Welding Society (AWS) Z49.1 Safety in Welding, Cutting, and Allied Processes, AWS Fact Sheet No. 1 Fumes and Gasses,  and AWS Fact Sheet No. 20 Thermal Spraying Safety Tips.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:           (a)   Major Metals Company, 844 Kochheiser Road, Mansfield, Ohio:  On or about October 23, 2014, the employer did not ensure that employees were provided with information on the hazards of the work place and the methods to protect from these hazards.  Information on the safe handling of combustible dust and the fire and explosion hazards associated with combustible dusts was not provided.
