29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a)  Major Metals Company on or about November 30, 2017, at the address of 844 Kockheiser Rd. Mansfield, OH: The employer failed to ensure the rotating sizing dies contained on the Tube Mill #1 Machine was adequately guarded. During a production run, an employee was exposed to amputations hazards after reaching across the sizing dies (point of operation) to apply grease to a bearing zerk. When performing this task, the operators hand/arm was within the proximity of the incoming nip point of the sizing dies.       Major Metals Company was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 956027, citation number 2, item number 7 and was affirmed as a final order on July 29, 2014, with respect to a workplace located at 844 Kochheiser Rd. Mansfield, OH.
