29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Moore Industries, Inc. - Montpelier, OH: On or about October 28, 2014, the employer did not ensure that employees were protected from amputation injuries while operating the Johnson V-24 band saw. The unused portion of the blade was left unguarded while cutting plastic parts.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. Moore Industries, Inc. - Montpelier, OH: On or about October 30, 2014, the employer did not ensure that Press 11, the Newbury injection molder (s/n 6648575S82), was adequately guarded to protect employees from point of operation hazards. Operators clearing plastic from the nozzle were exposed to the pinch points as the die and nozzle complete a cycle. b. Moore Industries, Inc. - Montpelier, OH: On or about October 30, 2014, the employer did not ensure that Press 13, the Newbury injection molder (s/n 4594175S80), was adequately guarded to protect employees from point of operation hazards. Operators clearing plastic from the nozzle were exposed to the pinch points as the die and nozzle complete a cycle.
