29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): On or about February 5, 2020, the employer did not ensure that containers storing hazardous chemicals such as, but not limited to cyclohexane, xylene, ethylbenzene, toluene, ethyl acetate, and ethyl alcohol which are included in 3M Adhesion Promoter 4298UV were labeled and marked with the product identifier, signal words, hazard statements, pictograms, precautionary statements and name, address and telephone number of the chemical manufacturer in that the only marking on the bottle identified it as Ad pro.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 5, 2020, the employer did not ensure that employees were protected from injury while operating the Dake Johnson V-24 band saw to cut plastic parts and cardboard in that the guard did not fully cover the unused portion of the blade with approximately 2.5 inches of the blade exposed above the point of operation.
