29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. On or about July 12, 2018, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as cleaning on the Cincinnati Press #6 (Model VT-550 54), and cleaning and setup activities on the Whittmann Robot on Press #6, and are exposed to amputation hazards from amputation and struck by hazards in that the equipment was not properly deenergized.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about July 12, 2018, employees were exposed to amputation injuries in that the Cincinnati #6 (Model VT-550 54) plastic injection molding press was not deenergized prior to performing cleaning activities. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. b.) On or about July 12, 2018, employees were exposed to struck by injuries in that the Wittmann (Model W433) robot on plastic injection molding press #6 was not deenergized prior to performing setup and cleaning activities. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished.
