Observed on or about 10/17/14.

29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use: a) Jobsite - The employer did not ensure that each job made ladder, used by employees, had ladder rungs that were uniformly spaced. As a result, employees were exposed to a fall hazard of approximately twelve (12) feet. Observed on or about 10/17/14.
