29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches:    (a) 431 General Way Mechanicsburg, PA - Employee(s) were using a six foot Warner stepladder to access the top plate board to set roof trusses on a residential home with a break in elevation of three feet from the top of the ladder to the top of the plate board, thereby exposing employees to injuries, on or about February 5, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 431 General Way Mechanicsburg, PA - Employee(s) were setting roof trusses on a residential home by walking the top plate board without the benefit of a fall protection system, thereby exposing employees to a fall from the top plate board of approximately 9 feet 2 1/2 inches to the ground below, on or about February 5, 2019.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
