29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Cincinnati Shear, #950 - The attached guard was out of adjustment allowing a 1/2 inch opening under the hold down feet. (b) Horizontal Band Saw, #215 - The supplied guard over the unused portion of the rotating blade did not extend the full width of the blade allowing the cutting teeth to protrude approximately 1/4 inch below the guard.

29 CFR 1910.217(c)(2)(i)(b): Point of operation guard(s) on mechanical power press(es) did not conform to the maximum permissible opening(s) specified in Table O-10: (a) Tie Department, foot operated, part-revolution mechanical power press, identification #604 - The installed guard where the worker inserted the part was worn and bent with an opening of approximately 7/8 inch. The distance to the danger point behind the guard was about 6 inches.
