29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. a. Manufactured Housing Enterprises, Inc. - Bryan, Ohio: On or about October 8, 2014, the employer did not ensure an engineering survey was conducted and recorded by a competent person prior to beginning demolition work on the roof and roof supporting members of Plant 1. An employee was exposed to a 12ft. fall to the ground when located on the damaged roof of Plant 1 to cut roof supporting members.

Manufactured Housing Enterprises, Inc. - Bryan, Ohio: On or about October 15, 2014, the employer did not ensure the scaffold being supported by the Gradall 534C-6 rough terrain lift did not extend more than 10 inches past the wheel base. The 18 foot platform extended past the wheel base approximately five feet on both sides.

Manufactured Housing, Inc. - Bryan, Ohio: On or about October 8, 2014, the employer did not ensure that employees who were tearing down damaged roofing materials were protected from falls. Employees were exposed to fall hazards of approximately 12 feet when performing the demolishing activities of the roof at Plant 1. Manufactured Housing Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(11), which was contained in OSHA inspection number 313782542, citation number 1, item number 2 and was affirmed as a final order on October 21, 2010, with respect to a workplace located at 09302 State Route 6, Bryan, Ohio 43506.
