Employees were exposed to caught-in and amputation hazards if the saw were to unexpectedly start while changing the machine blades.      b)  On or about November 13, 2014, the employer did not ensure that employees isolated energy sources on the Holzma (Model HCL-22) manufactured panel saw prior to engaging in blade changing activities.  Employees were exposed to caught-in and amputation hazards if the saw were to unexpectedly start while changing the machine blades.
