29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A)    Laminate Technologies, Inc., located at 161 Maule Road, Tiffin, OH 44883: On or about June 26, 2015, the employer did not develop and implement specific procedures to control hazardous energy of the equipment such as, but not limited to, the Laminators board coater rollers. Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before performing activities such as, but not limited to, cleaning the rollers. Employees were exposed to amputation hazards.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:     A)    Laminate Technologies, Inc., located at 161 Maule Road, Tiffin, OH 44883: On or about July 2, 2015, the employer failed to ensure that maintenance employees were provided with training on electrical safety-related work practices such as, but not limited to, conducting a test of the volt meter to ensure its proper operation and use of personal protective equipment for the hands, head, and body to protect against electrical hazards while working on or near energized or potentially energized electrical parts. One of the employees was exposed to 480 volts electrical hazards while checking fuses located in the plug box on the electrical bus bar.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     A)    Laminate Technologies, Inc., located at 161 Maule Road, Tiffin, OH 44883: On or about June 26, 2015, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, electrical and hydraulic were followed before employees conducted activities such as, but not limited to, cleaning the Laminators board coater rollers. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.     Laminate Technologies, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d)(3)), which was contained in OSHA Inspection # 1007922, Citation number 1, Item number 1, and was affirmed as a final order on 02/18/2015, with respect to a workplace located at 161 Maule Road, Tiffin, OH 44883.
