29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: a) Laminate Technologies, Inc. Tiffin, Ohio; On or about February 2, 2016, the employer did not ensure that employees verified hazardous energy sources such as, but not limited to, electrical and pneumatic were isolated and de-energized prior to conducting work such as but not limited to blade changes, cleaning, accessing the table, or opening the doors on the Holzma #2 HPL 570 Saw. Employees were exposed to injuries from unexpected start-up of the machine and energy release.
