Section 5.1(b) Containers when stored indoors shall not be located near exits, stairways or in areas normally used or intended for the safe exit of people:  a) On or about November 18, 2014, four LP Gas cylinders were stored around Assembly Area emergency exit door.

29 CFR 1910.178(q)(6): Industrial trucks were altered by the elimination of parts provided by the manufacturer:     a)  On or about November 18, 2014, the employer allowed the Clark Forklift to be operated with the engine compartment covers removed.    b) On or about November 18, 2014, the battery in the Clark Forklift was not secured nor was the battery sitting flat in the battery tray.     c) On or about November 18, 2014, the seatbelt was missing from the Clark Forklift.

The Ace Equipment Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 315288621, citation number 01, item number 002 and was affirmed as a final order on 07/07/2011, with respect to a workplace located at 1404 Smith Ave. Antigo, WI 54404.
