29 CFR  1910.252(b)(2)(i)(A):Helmets or hand shields shall be used during all arc welding or arc cutting operations, excluding submerged arc welding. Helpers or attendants shall be provided with proper eye protection.  On or about April 11, 2023, in the fabrication building of the establishment located at 810 Hudson Street Antigo, Wisconsin; employees performing initial tack welds were not utilizing helmets or hand shields exposing employees to eye injuries.  Exposure to ultraviolet light from welding can result in eye swelling, tearing and pain.  Welding without adequate eye protection may cause photokeratitis, conjunctivitis, cataracts, skin cancer, burns to retina and burns to dermis.

29 CFR  1910.252(b)(2)(iii):Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.  On or about April 11, 2023, in the fabrication building of the establishment located at 810 Hudson Street Antigo, Wisconsin; the employer did not ensure that welding work performed was enclosed by an individual booth, noncombustible or flameproof screens, or requiring the use of appropriate goggles to protect employees working in the adjacent area from ultraviolet radiation generated by welding, exposing employees to the hazards associated with ultraviolet radiation.

29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about April 11, 2023, at the establishment located at 810 Hudson Street Antigo, Wisconsin; the employer could not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.
