29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.    a.  On or about March 6, 2018, the employer did not ensure that an employee using a tight-fitting facepiece respirator while painting parts for balers and compactors was fit tested on an annual basis.  The employee paints for approximately 24 hours a week.

On or about March 6, 2018, the employer did not ensure that a makeshift fastener was not used to lift steel parts, motors, and frames of trash compactors and vertical balers.  The makeshift fastener was formed from a carabiner welded to a chain and used as a hook for lifting.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a.  On or about March 6, 2018, the employer did not ensure that all junction boxes had covers.  The junction box on the ceiling of the employee break room between the South wall and the bathrooms was missing a cover.

29 CFR 1910.305(g)(1)(iv)(A):   Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.    a.  On or about March 6, 2018, the employer used flexible cords as a substitute for the fixed wiring of a structure to power two retracting extension cord reels.  The extension cord reels were used to power hand tools when installing the final components on the compactors.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working with or around cranes lifting material were exposed to struck by hazards during the production of compactors and balers:    On or about March 6, 2018 employees were exposed to struck-by hazards while operating suspended loads in that the hook on the Lakeland Equipment Company 2 Ton crane #1 had a missing safety latch.    On or about March 6, 2018 employees were exposed to struck-by hazards while operating suspended loads in that the hook on the Abell-Howe 1 Ton crane #4 had a missing safety latch.    On or about March 6, 2018 employees were exposed to struck-by hazards while operating suspended loads in that the hook on the Abell-Howe 1 Ton crane #6 had a missing safety latch.    On or about March 6, 2018 employees were exposed to struck-by hazards while operating suspended loads in that the hook on the P&H 1 Ton crane had a missing safety latch.    On or about March 6, 2018, employees were exposed to struck-by hazards from potential load failure when working with a monorail crane that had extensive wear and damage on the load sprocket chain guide of the Budgit 2 ton crane #1.
