29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a) On or about 12/12/2014, four floor holes in the melt deck platform adjacent to the Charge Preheater were not guarded with standard railing or floor hole covers to prevent employees from walking into the openings.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) On or about 12/12/2014, two Molder employees were exposed to falls from greater than four feet above the ground level when the employees were working on an elevated cope with the working level approximately 4.9 feet above the ground level and no guard railing or fall protection in use.
