29 CFR 1910.179(j)(3): Periodic, complete, inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated in section 1910.179(j)(4). These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the items identified in sections 1910.179(j)(3)(i)-1910.179(j)(3)(x). Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard. a. On or about August 5, 2022, at the facility located at 1900 Enterprise Drive, De Pere, Wisconsin; the employer failed to complete the periodic inspection of the Pangborn Blast Machine Crane (Virginia Crane, Serial No.: VC12-1924) within 12 months from the most previous periodic inspection.

29 CFR 1910.179(l)(3)(i): Any unsafe conditions disclosed by the inspection requirements of paragraph (j) of this section shall be corrected before operation of the crane is resumed. Adjustments and repairs shall be done only by designated personnel. On or about August 5, 2022, at the facility located at 1900 Enterprise Drive, De Pere, Wisconsin; the employer failed to correct unsafe conditions identified on the Pangborn Crane (Manufacturer: Virginia Crane, Serial #: VC12-1924) such as, but not limited to bridge bumpers were dry-rotted and trolley bumpers that were dry-rotted.

29 CFR 1910.184(c)(6): Slings shall be securely attached to their loads. On or about August 5, 2022, at the facility located at 1900 Enterprise Drive, De Pere, Wisconsin; employees were exposed to struck by and crush hazards while moving castings using a four-leg alloy steel lifting chain (Serial Number: 00013553-1) in a manner that did not prevent accidental load disengagement in that the chain sling was not equipped with any safety latches.
