OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards while operating the polyethylene foam densifier.    a. IVEX Protective Packaging Inc. located in Sidney, Ohio:  On or about January 8, 2015, employees operating the polyethylene foam densifier located in the production area of the facility, were exposed to fire and explosion hazards from the accumulation and ignition of isobutane gases inside of the equipment.    Among other methods, one feasible and acceptable method to correct this hazard includes, but is not limited to: 1. Ensure raw material is sufficiently pre-processed to remove all isobutane or other flammable gases prior to placement in the densifier; 2. Provide sufficient ventilation to the densifier to ensure flammable gas concentrations do not exceed the lower flammable limit; 3. Eliminate all possible sources of ignition in the densifier, including static electricity.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there was entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toe board wherever, beneath the open sides.    a. IVEX Protective Packaging Inc. located in Sidney, Ohio:  On or about January 8, 2015, the employer did not ensure the work platform surrounding the densifier was equipped with a mid-rail, exposing employees to a fall hazard of 55 inches.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a. IVEX Protective Packaging Inc. located in Sidney, Ohio:  On or about January 8, 2015, the employer did not ensure employees operating the densifier were wearing flame retardant clothing to protect the torso and arms.    IVEX Protective Packaging Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.132(a), which was contained in OSHA inspection number 850603, citation number 1, item number 18 and was affirmed as a final order on August 12, 2013, with respect to a workplace located at 456 S. Stolle Drive, Sidney, Ohio.
