29 CFR  1910.119(g)(1)(i):The employer did not train each employee in an overview of the process:  IVEX Protective Packaging located at 440 Stolle Avenue in Sidney, Ohio 45365:  On or about February 14, 2023 the employer did not ensure that each employee involved in process safety management operations were trained in the specific safety and health hazards, emergency operations, and safe work practices applicable to the employee's job tasks. Employees operating and maintaining the foam extrusion lines were not properly trained in the steps of each operating phase, operating limits, safety and health considerations, and the safety systems and their functions. Employees were exposed to isobutane hazards such as, but not limited to, frostbite, deflagration, and explosion.

29 CFR  1910.119(g)(3):The employer did not ascertain that each employee involved in operating a process received and understood the training required by this paragraph:  IVEX Protective Packaging located at 440 Stolle Avenue in Sidney, Ohio 45365:  On or about February 14, 2023 the employer did not ensure that a record was maintained which contains the identity of the employee, the date of the training, and the means used to verify that the employee understood the training. Employees operating and maintaining the foam extrusion lines were not properly trained in the steps of each operating phase, operating limits, safety and health considerations, and the safety systems and their functions. Employees were exposed to isobutane hazards such as, but not limited to, frostbite, deflagration, and explosion.

The documentation shall identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test.  IVEX Protective Packaging located at 440 Stolle Avenue in Sidney, Ohio 45365:  On or about February 14, 2023 the employer did not ensure documentation of inspections on process equipment, such as but not limited to the oxidizer fan, gas train, ductwork and piping for the extrusion line, included the date of the inspection, name of the person performing the inspection, equipment identifier, and the results of the inspection. Employees were exposed to isobutane hazards such as, but not limited to, frostbite, deflagration, and explosion.

29 CFR  1910.307(c):Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  IVEX Protective Packaging located at 440 Stolle Avenue in Sidney, Ohio 45365:  On or about February 14, 2023, the employer did not ensure electrical equipment was approved for the hazardous location they were operated in. Employees were exposed to isobutane hazards such as, but not limited to, fire, deflagration, and explosion in that a non-approved floor fan was being operated in the environmental room of the foam extrusion line within the NEC Class I Group D Division II area.
