The employer did not ensure employees were trained in the purpose and function of the energy control program, with regards to the mechanical, electrical, and pneumatic energy sources within FAP s.r.l. winder, during machine set up process, maintenance and repair.  Ivex Protective Packaging Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(c)(7)(i), which was contained in OSHA inspection number 1497749, citation number one, item number two and was affirmed as a final order on January 15, 2021 with respect to a workplace located at 2560 Campbell Rd., Sidney, OH 45365. Stolle Dr. Sidney OH 45365, that operated the Line Three foam extruder line winder, were exposed to in-going nip points, struck-by and caught-in hazards in that the Haanen Packard Machinery winder (S/N: 551179.6) was not locked out prior to employees placing any part of his or her body where an in-going nip point exists while clearing jams, cutting loose material during a jam and misfeed of the winder. During this instance employees were exposed to in-going nip points at speeds of 215 feet per minute. The employer did not ensure that energy control application steps to control the mechanical, electrical, and pneumatic energy sources were implemented prior to servicing tasks.  Ivex Protective Packaging Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d), which was contained in OSHA inspection number 1497749, citation number one, item number one and was affirmed as a final order on January 15, 2021 with respect to a workplace located at 2560 Campbell Rd., Sidney, OH 45365.
