29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel that never exceeds one-fourth inch: (a) Building 250, Sheet Metal Shop: On or about January 15, 2015 and times prior thereto, Rockwell Bench Grinder, Model: 23.902 and Serial Number W11-8323-902 was not provided with a tongue guard that was ¼ of an inch from the abrasive wheel on the right and left sides. Employees were exposed to foreign particles embedded to body and eyes.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. (a) Building 250, Composite Fiberglass Shop; On or about January 15, 2015 and times prior thereto, approximately seven (7) foot flexible extension cord, Carol 12/3 Water Resistant SOOW CSA, servicing portable lights and the extension cord at the male end of the attachment plug was not connected to a device and fitting so that strain relief was provided. The cord was used daily and employees were exposed to electrical shock.
