29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) 33 State St., Springfield, MA: On or about January 13, 2015, a stairway or ladder was not provided for employees to enable access over a 4 foot tall fence to reach an extension ladder that was used to reach the ground level which was nearly 17 feet below.
