29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:     a. Site- Employees working from the roof were not protected from a fire hazard where a fire extinguisher was not located on the roof. Multiple employees were smoking cigarettes while applying flammable adhesives on the roof surface.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. Roof- Employees working from the upper roof were exposed to a fall of approximately 27 feet where personal fall arrest systems or a combination of warning line system and safety monitoring system was not in use while employees were installing a rubber membrane roof.    Sun Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(11), which was contained in OSHA inspection number 1096740, citation number 2, item number 1 and was affirmed as a final order on December 23, 2015, with respect to a workplace located at 380 Union Street, West Springfield, MA 01089.    Sun Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.132(a), which was contained in OSHA inspection number 313208746, citation number 01, item 01 with respect to a workplace located at 80 Damon Road, Northampton, MA 01105.    Sun Roofing Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 313204232, citation number 01, item number 01 with respect to a workplace located at 485 Appleton Street, Holyoke, MA 01040.
